恙虫病相关肺炎与社区获得性肺炎的临床特征及实验室指标比较

比较恙虫病相关肺炎与社区获得性肺炎(CAP)的临床特征,评价乳酸脱氢酶(LDH)的辅助鉴别价值。回顾性纳入临床诊断恙虫病相关肺炎113例及同期随机选取的CAP患者110例,比较临床和实验室指标,并行ROC分析。结果显示,恙虫病相关肺炎组发热、肌痛更常见,血小板、白蛋白和血钠较低,肝酶、LDH、CRP和PCT较高。213例有LDH结果;LDH的AUC为0.937(95%CI:0.904~0.970),截断值282 IU/L时灵敏度89.6%、特异度85.0%。恙虫病相关肺炎呈血小板减少、低白蛋白血症及组织损伤指标升高,LDH可辅助鉴别,但不能替代病原学检查和综合判断。
