恙虫病相关肺炎与社区获得性肺炎的临床特征及实验室指标比较

摘要

比较恙虫病相关肺炎与社区获得性肺炎（CAP）的临床特征，评价乳酸脱氢酶（LDH）的辅助鉴别价值。回顾性纳入临床诊断恙虫病相关肺炎113例及同期随机选取的CAP患者110例，比较临床和实验室指标，并行ROC分析。结果显示，恙虫病相关肺炎组发热、肌痛更常见，血小板、白蛋白和血钠较低，肝酶、LDH、CRP和PCT较高。213例有LDH结果；LDH的AUC为0.937（95%CI：0.904～0.970），截断值282 IU/L时灵敏度89.6%、特异度85.0%。恙虫病相关肺炎呈血小板减少、低白蛋白血症及组织损伤指标升高，LDH可辅助鉴别，但不能替代病原学检查和综合判断。
